In order to receive your rebate, you must follow these steps:
- Undergo a Recent Eye Exam: The product must be purchased within 90 days after an eye exam and proof of the eye exam, with receipt, is required.
- Complete Online Form: Visit http://www.acuvue.com/buying-rebates-launch386-160.htm and complete the online rebate form. REBATE TERMS & CONDITIONS:
For purchases made between 1/01/2012 and 09/30/2012
Purchases of 1-DAY ACUVUE® MOIST®, 1-DAY ACUVUE® MOIST® for ASTIGMATISM, 1-DAY ACUVUE® TruEye®, ACUVUE® OASYS®, ACUVUE® OASYS® for ASTIGMATISM and ACUVUE® OASYS® for PRESBYOPIA must be made 1/1/12 through 09/30/12 and rebate requests received at the mailing address on or before 10/31/12**. Product purchase must be made within 90 days after eye exam. Limit one product rebate per customer, per offer, per ACUVUE® Brand purchase, per yearly eye exam visit. You must enroll in the ACUMINDER® Tool, a free contact lens-change reminder service (after initial enrollment you may cancel at any time and still receive rebate). This offer not valid in combination with any other product offer or rebate including Money Back Guarantee. Offer valid for U.S. residents only. Offer not valid where prohibited by law.
